School Physical Information

Preschool students who will be entering Oglesby Public School for the first time will be required to have a physical examination and immunizations in accordance with the requirements of the State of Illinois. Lead screening is also required for preschool students; please submit the result of the lead test with the physical examination.

Students who will be enrolling in Kindergarten will be required to have a physical examination and immunizations in accordance with the requirements of the State of Illinois. There is a state law requiring eye exams for school-aged children that took effect on January 1, 2008. The law calls for a comprehensive eye exam to take place within one year prior to entering kindergarten. Proof of the eye exam must be submitted by October 15 of each school year. Kindergarten students also need a dental exam in accordance with Law HB 752. Lead screening is only required for Kindergarten students if no previous screening was done in a preschool program. If you had a previous lead screening, please submit the results, if these are not already on file at Oglesby.

Students who will be entering in the Sixth Grade are required to have a physical examination with updated immunizations along with a dental examination.

Sports physicals are needed for students playing sports in grades 5, 7, and 8. The school physical for 6th graders will be accepted as their sports physical. Please note a sports physical will not be accepted as a school physical for 6th grade students.

It is the parents’ responsibility to make sure these requirements are met prior to the start of the school year. Your child will be excluded from school upon review of his or her records if they are found to be incomplete. Please hand in your child’s physical, dental, and eye examinations at registration on July 30, 2024

You can obtain a school physical with your primary care physician or by calling the Community Health Partnership of Illinois/Mendota at (815) 539-6124. Dental exams can be made by calling your family dentist or by calling the Community Health Partnership of Illinois/Mendota at (815) 539-6124. Sports physicals may be obtained through your family physician or at local prompt care centers (including OSF On Call Urgent Care in Ottawa and OSF Prompt Care in Peru, Ottawa, or Princeton). Please call School Nurse Jordan at (815) 883-8932 ext. 118 if you have any questions regarding these requirements.
